Transcranial direct current stimulation: historical essay and neurophysiological mechanisms of action

The article describes the main stages of development of transcranial direct current stimulation since 1789, and organizes data in the literature on the neurophysiological mechanisms of action of the method. It emphasizes the special role of Russian scientists in the development of this method (starting from the 70-ies). It has been shown that in the process of transcranial direct current stimulation changes are registered not only locally in the underelectrode space, but also in the symmetric areas of the opposite hemisphere and deep brain structures (for various cortical areas, a certain selectivity of descending influences on subcortical structures is characteristic). Arguments in favor of strengthening self-regulation mechanisms in the process of treatment by this method (changing of the excitability of the cortex, changes in microcirculation, metabolism, etc.) are given. A possible mechanism of action of the method is the direct effect of electric fields on neuronal processes of the brain.
